Certain medical documents are required at the time of registration. A current physical examination performed within 365 days from the child’s first day of school in the district and proof of NJ minimum vaccination is required for all new students.
Required Medical Documentation:

Must be signed, initialed, or stamped by your doctor or a copy of an A45 Card from previous New Jersey school. The following immunizations are required by law for all students entering school at age five:
For Preschoolers: age appropriate immunizations as mentioned above AND
If your child will be receiving a physical exam and/or new immunizations after registering to start school in September, please have the documents scanned and emailed to the nurse no later than the last Friday before school closes in June for the summer.
If your child will be receiving a physical exam and/or new immunizations this summer (for June through September birthdays only), you must provide us with a written note from your pediatrician by the last Friday before school closes in June, stating when your child is scheduled for their physical examination and/or immunizations.
If your child will be starting or transferring into school during an in session school year, the enrollment physical is due within 30 days of registering in the EG district, also acceptable is a copy of the NJ A45 card with an enrollment physical from a NJ school.
Children without the required medical paperwork / vaccinations as listed above will not be able to attend the first day of school.

All communication after the end of the school year in June, should take place via email only.

- EG school form or also acceptable is the Child Universal Health Report issued by your physician’s office. The district cannot accept a printout of a Visit Summary. Form must be completed and signed or stamped by a physician.

- ▪ 4 DTaP – one dose given on or after 4th birthday or any 5 doses
- ▪ 3 Polio – one dose given on or after 4th birthday or any 4 doses
- ▪ 2 Measles, Mumps, Rubella (MMR)
- ▪ 3 Hepatitis B
- ▪ 1 Varicella Vaccine after 1st birthday or documented proof of the disease from doctor or parent
For Preschoolers: age appropriate immunizations as mentioned above AND
- ▪ 1 HIB Vaccine required on or after 12 months
- ▪ 1 Pneumococcal Vaccine required on or after 12 months
- ▪ Annual Influenza Vaccine required by December 31st

The East Greenwich Report of Physical or the Universal Child Health Record are the only documents the school district will accept as proof of physical.
