​Dear Parents/Guardians:                                                                                                       
Welcome to the new school year! We hope you had a relaxing, healthy, and exciting summer vacation.  With the start of the new school year, we ask that you alert the school nurse of any changes to your child’s health, list emergency contact information, and give permission for the nurse to screen your child during the school year by completing this Health History Update and Permissions form. If you do not wish to have your child’s screening completed, please send me your refusal in writing by September 15 and please forward a copy of your child's annual physical results once performed. Thank you in advance for your assistance in keeping your child’s school health records current.
Most health-related information may be found on this website. Please refer to it throughout the year for health information, registration information, and required medication dispensing forms, including asthma action plans, allergy action plans, seizure action plans, etc. 
  • If your child will need medication administered during school hours, please review our medication guidelines. 
  • To administer medication in school, a doctor’s order will be required on an East Greenwich Township school form. Your doctor will not have our school forms. Please use this link to print the required East Greenwich Medical Dispensing Forms and /or Emergency Action Plans before you head to the doctor.
  • From time to time, children have accidents, spills, or torn clothing.  We try to provide a change of clothes when possible.  If your child is prone to accidents, please provide them with a complete change of clothing. Please return any clothing provided for your child after laundering it in a zip-lock bag labeled for the nurse. 
  • Please be aware, the school is not responsible for the assessment/treatment/follow-up of injuries occurring at home or outside of school property. 
  • A physician’s note is required for any student that is not able to participate in physical education (PE) class. If the physician’s note does not have a date noted to return to full activity, the restriction will remain in effect until the physician gives clearance in writing to return.  No exceptions.  Please ask for a documented return to play date when applicable. 
All students transferring into the district should be enrolled with a current physical (within 365 days) and up-to-date vaccinations or a valid medical or religious exemption per N.J.A.C. 8:57-4. The influenza vaccine is required for all of our PreK students under the age of 5 annually by December 31.
General Guidelines Regarding Exclusion from School: 
Please alert the school nurse if your child is diagnosed with any illnesses or communicable diseases such as COVID-19, flu, strep throat, pink eye, chicken pox, head lice, MRSA, impetigo, ringworm, etc. The school nurse should be advised so the necessary steps can be taken. Please refer to the NJ Guidelines for the Control of Outbreaks in School and Child Care Settings School Exclusion List.
  • Students with a fever greater than 100.0 degrees will need to remain at home, as per school policy, until after the resolution of the fever without fever-reducing medications for 24 hours. 
  • Students with vomiting or diarrhea must be symptom-free for 24 hours before returning to school.
  • When students return to school following an illness, they are expected to join in all activities, unless specifically excluded in writing by their physician. If they are not well enough to go outdoors, then they should stay home another day or so. 
  • A doctor’s note is required after 3 days of consecutive absence when ill.
  • Please refer to the Parent Handbook regarding absences.
